1ST STEP Application for Belize Normal Provisional Registration Procedure

1.Request for confirmation of availability of vessel’s name

2.Application for registration of ships form ,see the following:

1)APPLICATION FORM FOR REGISTRATION-

2)APPLICATION FOR MARITIME SHIP STATION LICENCE- SSAS

3)MANNING APPLICATION

4)AMENDMENT OF CONTINUOUS SYNOPSIS RECORDS

5)CSO APPLICATION FORM(ISPS CODE)

6)ISM Forms

7)APPLICATION FOR ISPS

8)POWER OF ATTORNEY

9)LETER OF APPOINTMENT-SHIP AGENT

3.Copy of Proof of ownership in the form of Bill of Sale Signed and Notarized (where applicable ）

4.If new ship , a copy of Builder’s certificate signed and Notarized

5.Power of Attorney appointing a shipping agent in Belize --Signed and Notarized

6.Valid Cargo Ship Radio Certificate in compliance with GMDSS Code (where applicable)

7.International Tonnage Certificate (where applicable)

8.Proof of compliance with ISM Code-Copy of DOCUMENT OF COMPLIANCE and SAFETY MANAGEMENT CERTIFICATE (where applicable)

9.Radio Accounting Authority Letter ( RAA ) (where applicable)

10.Applicant's Passport / ID Card & Certificate Of Incorporation

2ND STEP Application for Belize Normal Permanent Registration Procedure

Please compile and submit the following document to our office minimum 2 months before the respective expiry of the Provisional Registry Certificates .

1.ORIGINAL or certified true copy of a dully NOTARIZED BILL OF SALE, judical sale instrument or Builder certificate

2.ORIGINAL DELETION CERTIFICATE from previous registry

3.ORIGINAL or certified true copy of POWER OF ATTORNEY appointing a shipping agent in Belize

4. Copies of all valid statutory certificates issued by a Regonized Organization on behalf of the government of Belize, including the ITC, DOC, SMC,ISSC, CSSRC, etc.

5. Letter from Radio Accounting Authority

6. Copy of most recent crew list and copies of CRA’s

7. Copy of Bunker Certificate & Wrecks Certificate ( Certificate must be valid ）

8. LRIT Test Report (where applicable)
