1.Request for confirmation of availability of vessel’s name

2.Application for registration of ships form ,see the following:

3）MANNING APPLICATION

4）AMENDMENT OF CONTINUOUS SYNOPSIS RECORDS

5）CSO APPLICATION FORM(ISPS CODE)

6）ISM Forms

7）APPLICATION FOR ISPS

8）POWER OF ATTORNEY

3. Copy Notarized of Bareboat Charter Contract between Owner & Charterer. .

4. Copy of the consent from the Mother Registry thereby authorizing the parallel registration under the Belize Registry.

5. A copy of an official document proving Ownership in the form of a Certificate of Registry or Certificate of Ownership issued by the Chinese Administration.

6. A copy of a Certificate of Liens & Encumbrance issued by the Mother Registry is also required.

7. Power of Attorney executed by the Owner thereby appointing a Shipping Agent in Belize must be submitted duly notarized.

8.Copies of any previous statutory certificates issued to the vessel including a copy of the ITC, ISSC & vessel’s complete CSR File.

9.Radio Accounting Authority Letter ( RAA ) （where applicable）
