List Required Documents for Hong Kong COE Application

一、Period of validity for Provisional COE is three months

a. Our company can assist to fill in M0713A form then transfer the WORD format into PDF format

b. Scan copy of Passport

c. Scan copy of STCW 2010 Certificate of Competency(COC),should be valid for at least 3 months

d. Scan copy of STCW 2010 GMDSS( if any),should be valid for at least 3 months

e. Scan copy of STCW 2010 Dangerous Cargo Endorsement(DCE,if any),should be valid for at least 3 months

Subject to the documents are in order, the 3-month valid CRA will be available that day.

二、Period of validity for Permanent COE keeps the same as certificate issue by local MSA(belowing required documents shoud be printed out then courier them to Hong Kong by us)

f. Our company can assist to fill in the M0713 form,the photo should be pasted in the photo box

g. Copy of CRA

h. Copy of passport with our company signature and applicant signature

i. Copy of STCW 2010 Competency of Certificate(COC) with our comany signature and applicant signature

j. Copy of  medical certificate with our company siganature and applicant signature

k. Copy of STCW 2010 GMDSS (if any) with our company signature and applicant signature

l. Copy of STCW 2010 Dangerous Cargo Endorsement (if any) with our company signature and applicant signature
